Have you, or has your child, moved from children’s NHS or Social care into adult services in this last year? Please share your experience.

If you have moved between services in the last year – whether the service has been providing physical, mental or emotional support – we would like to hear about your
experience.

Where will this feedback go?

The results from this feedback will be shared with the Suffolk and North East Essex Integrated Care system. If you live in Waveney, we will share your responses with the Waveney Primary Care Networks. This information will help providers, and commissioners to better understand the concerns people have, and what support people need, when moving between services.
