Moving from Child to Adult Services – Share your experiences
Have you, or has your child, moved from children’s NHS or Social care into adult services in this last year? Please share your experience.
If you have moved between services in the last year – whether the service has been providing physical, mental or emotional support – we would like to hear about your
experience.
- Were you given information and support to help you plan for the change?
- How did the move to a different service affect you?
- What worked well, and what could have been improved about your experience?
If you’ve got an experience to share, the best way to share it is by completing our short survey (we do not ask for your name when completing this survey). This is the link:

If you need support to complete the survey, or if you need it in another format, please contact us on freephone 0800 448 8234 (you will not be charged for the call). To share your views with us, you need to be over the age of 16.
Where will this feedback go?
The results from this feedback will be shared with the Suffolk and North East Essex Integrated Care system. If you live in Waveney, we will share your responses with the Waveney Primary Care Networks. This information will help providers, and commissioners to better understand the concerns people have, and what support people need, when moving between services.
