YOUR
RIGHTS AND RESPONSIBILITIES
As a patient you have the right of access to your doctor or other clinician
during normal surgery hours and appropriate medical services and treatment
will be provided to you as set out in this leaflet. Appointments will normally
be made for you within two working days. If you need to see a doctor or
one of our nursing team on the same day because of an urgent problem then
you will be seen. If an appointment has been made for you that you no longer
need, please telephone the surgery to cancel so that the appointment may
be given to another patient. Treatment is provided by members of our clinical
team in full consultation with you and with your consent.
The practice operates a zero tolerance policy. If a patient is violent
or abusive, we reserve the right to ask them to leave the premises and
to remove their name from our practice list.
PATIENT CONFIDENTIALITY
The practice will ensure that patient confidentiality is maintained at
all times by all members of the practice team. We ask patients to appreciate
that their healthcare is provided by a multidisciplinary team and in
order for patients to benefit from the best possible healthcare, it is
sometimes necessary to share patient information with other team members.
We will only divulge patient information direct to patients or, in the
case of a child under 16 years of age, to a parent. If any patient is
happy for their medical information to be given to another person, please
ask in reception for a form of authority, which should be completed and
signed by the patient and returned to the surgery.

MEDICAL AND
NON-MEDICAL STUDENTS AND TRAINING
We are not a designated teaching practice although we sometimes have
students in the practice who are either in nurse training or are hoping
to study medicine. Your doctor or nurse will always ask for your permission
before consultations where a student is present. To help the doctors
with their continuing education, you may be asked if you are happy for
your consultation to be videotaped.
COMMUNITY HEALTH STAFF
We work very closely with our community health team including District
Nurses, Health Visitors, Midwives, Physiotherapists, Occupational Therapists,
and Podiatrist.

COMPLAINTS /
COMMENTS
We welcome patients’ comments about the service received from practice
members.
If you wish to make a complaint about the services you have received
please ask at reception for a leaflet or ask to speak to Mrs Alison Arnold,
the Staff & Patients Manager.
